Most of the administrative forms to be used by McMaster graduate students at various stages of their program are available from the School of Graduate Studies. Other forms are listed below.

 

MS Word template. The CV information will be used by the department as part of the nomination of graduate students for external and internal scholarships. Students should submit an updated printed copy to the department as appropriate and at least once a year, in September.

PDF format. Sections of the report must be completed by students prior to a Supervisory Committee Meeting.

Form to be completed for the final submission of an MSc thesis to the School of Graduate Studies.

  • PhD Defence Report

This form will be provided directly to the Defence Chair by the School of Graduate Studies.

Form to be completed for the final submission of a PhD thesis to the School of Graduate Studies.
